The Role

The Supports Coordinator, Aged Care provides care coordination and support to people who are aged and may be living with a disability or mental health issues.


The Supports Coordinator has the following key areas of responsibility:

  • Undertaking initial assessments and reviews with customers to co-design their service plans and budgets
  • Providing care coordination
  • Building and managing stakeholder relationships
  • Problem-solving and escalating identified risks
  • Ensuring continuous improvement of work practices through audits and participation in specific projects

About You

  • Proven experience in case management or support coordination, including strong case load management skills
  • Thorough understanding and demonstration of person-centred approaches and principles of inclusion
  • High level ability to undertake thorough and sensitive assessment of people with complex needs
  • Demonstrated ability to work with customers with complex needs to develop and progress towards goals
  • Demonstrated ability to acquire and apply knowledge of statutory requirements
  • Detailed working knowledge of community networks and services
  • Demonstrated written ability including report writing and case noting
  • Demonstrated capacity to adapt skills and knowledge to meet a range of customer needs across different program types
  • Computer literacy (Office suite and capacity to use cloud based software)



  • Proven experience working in fee for service or billable hours environments and ability to prepare and work within customer budgets
  • An understanding of statutory requirements relevant to the Aged Care sector
  • An area of specialty including but not limited to aged health, aged housing, disability and/or mental health


You will have:

  • Minimum Certificate IV in Community Services or equivalent, plus substantial proven experience working with people with complex needs, including but not limited to aged care, disability and/or mental health



  • Diploma or degree in allied health, social work, rehabilitation counselling, community services, developmental education or disability
  • Specialist qualification including but not limited to aged care, disability and/or mental health


This role is based at our Central Office in Bowden however some off-site work will be required.
