We deliver social services that are shaped around individual needs and goals to help create positive futures built on independence and esteem.  We deliver services in Aged Care, Child & Family, Financial Health, Mental Health & NDIS.  We operate two social enterprises, a community child care centre and a psychology practice.  We have been in operation for over 150 years.

The Role

The Supports Coordinator, Aged Care provides care coordination and support to people who are aged and may be living with a disability or mental health issues. Their primary purpose is to support these customers to remain living in the community with dignity by providing person-centred care that focuses on their strengths, interests and abilities.

 

The primary responsibilities of the role are to:

  • Provide person-centred care coordination/case management to Home Care Package (HCP) customers.
  •  Undertake initial assessments and reviews with customers to co-design their service plans in line with budgets.
  •  Build and manage effective stakeholder relationships.
  •  Participate in continuing professional development to maintain, develop and enhance knowledge of best practice.

About You

You will have experience working in community services, particularly in working with older people and people with complex needs. Along with your person-centred and sensitive approach, you will be someone with a genuine passion for improving the lives of older people.

Qualifications and experience

You will have:

  • Relevant qualifications in allied health, community services or social services, plus substantial proven experience working with older people and/or people with complex needs.
  • Proven experience in complex case management or support coordination, including strong case load management skills.
  • High level ability to undertake thorough and sensitive assessment of people with complex needs.
  • Demonstrated ability to work with customers with complex needs to develop and progress towards goals.
  • Demonstrated ability to acquire and apply knowledge of statutory requirements.
  • Detailed working knowledge of community networks and services.
  • Demonstrated written ability including report writing and case noting.
  • Computer literacy (Office suite and capacity to use cloud based software).
  • A current South Australian Driver’s Licence
  • A current DHS Aged Care Sector Employment Screening and Working With Children Check, or ability to obtain at your own cost prior to commencement of employment.
  • Under various Emergency Management Directions currently in place in South Australia, all staff, volunteers and students who undertake work at any UCWB site or who undertake in-home and community aged care and disability support work are required to be vaccinated against COVID-19.

Additional prior experience working in a fee for service or billable hours environment, and ability to prepare and work within customer budgets would be an advantage.

Location

This role is based at Bowden just a short walk to the popular Plant 3 and Plant 4 entertainment complex in Gibson Street however off-site work out in the community will be required.
